How Acupuncture and Pharmacopuncture Work for Elbow Pain

Why acupuncture works for elbow pain

The effectiveness of acupuncture for elbow tendinopathy has been demonstrated by multiple clinical studies. Needle stimulation does not simply block pain — it produces a complex set of actions that change the local microenvironment and promote tissue recovery.

Mechanisms of acupuncture

When the needle is inserted into tissue, a fine physical stimulus is generated, triggering a series of physiological responses.

  • Increased blood flow: Blood vessels around the needle dilate and local blood flow increases. Because tendon attachments are inherently poorly perfused, this effect is especially important.
  • Pain signal blocking: Needle stimulation activates A-beta nerve fibers and inhibits pain signals carried by C-fibers (gate control theory). It also promotes the release of endorphins in the brain.
  • Muscle relaxation: Acupuncture at trigger points elicits a local twitch response (LTR) that releases muscle knots.
  • Anti-inflammatory effect: Needle stimulation activates a systemic anti-inflammatory response through the vagus nerve (cholinergic anti-inflammatory pathway).

Pharmacopuncture: combining the needle and the medicine

Pharmacopuncture is a method in which extracts purified from medicinal herbs are injected directly into acupoints or lesions. Because the physical stimulus of the needle and the pharmacological action of the herbs occur simultaneously, a synergistic effect can be expected. For elbow tendinopathy, herbal combinations with anti-inflammatory and blood-activating properties are mainly used.

Bee venom: the anti-inflammatory action of apitoxin

Bee venom therapy is a treatment in which purified bee venom extracted from the venom sac is injected into acupoints. Melittin, the main component of bee venom, has potent anti-inflammatory action, and its effects on chronic musculoskeletal disorders such as tendinopathy have been reported.

  • Melittin: Anti-inflammatory action via PLA2 inhibition
  • Apamin: Neuroprotective and analgesic effects
  • Precaution: A bee venom allergy test must be performed beforehand.

Treatment cycle and progress

In the acute phase, acupuncture and pharmacopuncture are given 2–3 times per week, and the frequency is reduced to once a week as symptoms improve. Meaningful improvement is usually experienced after 4–8 weeks of treatment, and chronic cases may require 12 weeks or more.
