Chuna Therapy — Principles and Effects of Spinal Correction

What is chuna therapy

Chuna therapy (推拿療法) is a Korean medicine manual treatment in which a Korean medicine doctor uses the hands, parts of the body, or auxiliary tools to correct structural imbalances of the spine, pelvis, joints, and fascia. Since 2019 it has been covered by national health insurance, greatly reducing patient cost, with up to 20 sessions per year covered. From an evidence-based medicine perspective, the effectiveness of chuna therapy for low back pain has been confirmed by numerous clinical studies.

Major types of chuna

  • Joint chuna (HVLA): A high-velocity, low-amplitude technique that corrects within the normal range of joint motion. A "pop" sound may occur during treatment, which is cavitation — gas bubbles in the joint cavity bursting.
  • Myofascial chuna (MFR): A gentle technique that releases tension and adhesions in muscles and fascia. Because it causes less pain, it is suitable for acute-phase or elderly patients.
  • Craniosacral chuna (CST): A delicate technique that senses and corrects the subtle rhythm of the cranium and sacrum to release dural tension. It is also used for headache and TMJ disorders.
  • Flexion-distraction technique: A method that flexes and distracts the lumbar spine on a special table to lower intradiscal pressure, effective for disc herniation and stenosis.

Treatment process

Chuna treatment proceeds after a systematic assessment.

  • Assessment: Posture analysis, joint range-of-motion testing, and palpation are used to identify the location and direction of subluxation.
  • Treatment: The technique appropriate for the identified segment is applied. One session takes about 15–20 minutes, with multiple regions corrected sequentially.
  • Post-treatment care: Acupuncture, moxibustion, and cupping are combined after correction to release tension in surrounding muscles and maintain the corrected state.

Insurance coverage and treatment cycle

With national health insurance coverage, the patient out-of-pocket cost per chuna session is approximately 7,000–12,000 KRW, making it affordable. In the acute phase, sessions are scheduled 2–3 times per week; during the stable phase, the interval is widened to once per week, typically as part of an 8- to 12-week treatment program. To maintain correction long-term, postural correction habits and core exercises must be performed alongside.
