Why does the same diet produce different results
Most people have experienced this: with the same diet and exercise, some lose weight while others stay the same. Korean medicine explains this through differences in constitution (體質). Sasang medicine (四象醫學) classifies people into four constitutions — Taeeumin (太陰人), Soeumin (少陰人), Soyangin (少陽人), and Taeyangin (太陽人) — and each has different metabolic traits, fat-storage patterns, and food compatibility.
Obesity traits and tailored strategies by constitution
- Taeeumin (太陰人): About 50% of the population — the most common type. Their organ structure features a large liver and small lungs, with high nutrient absorption and weak dispersal. They gain weight easily and tend to accumulate visceral fat. Tailored strategy: aerobic exercise that produces ample sweat, restriction of wheat and oily foods, and intake of diuretic foods like Job's tears, coix seed, and seaweed. The herbal prescription is a modified Taeeumjowi-tang (太陰調胃湯).
- Soeumin (少陰人): Large kidneys and small spleen. Weak digestion, cold hands and feet; they don't gain weight easily, but when they do, the lower body tends to be heavier. Tailored strategy: warm foods, small frequent meals, and use of ginseng and ginger. Yoga and walking are preferred over intense exercise. The herbal prescription is a modified Hyangsayangwi-tang (香砂養胃湯).
- Soyangin (少陽人): Large spleen and small kidneys. They run hot and have a quick temper, with a tendency toward upper-body obesity (face and arms). Tailored strategy: cooling vegetables and fruits, with barley and mung-bean rice recommended. Heat-dispersing exercises like swimming and cycling suit them. The herbal prescription is a modified Hyeongbangdojeoksan (荊防導赤散).
- Taeyangin (太陽人): A very rare constitution with large lungs and small liver. Obesity itself is rare, but the liver is easily damaged by oily foods. A diet centered on lean seafood and vegetables is recommended.

An accurate constitutional assessment is performed by a Korean medicine doctor through pulse diagnosis, interview, and observation of body type. A diet that does not match your constitution will not only be ineffective but may also harm your health, so consulting a specialist first is recommended.
