Andropause — Restoring Energy from Vitality Loss and Hormonal Change with Korean Medicine

What andropause is

Men also experience a midlife transition. From the mid-40s, testosterone declines by 1–2% per year, and fatigue, decreased libido, loss of muscle strength, abdominal weight gain, low mood, and sleep disturbance appear gradually. Medically this is called late-onset hypogonadism (LOH) and is diagnosed by free testosterone levels. Unlike female menopause, the change is gradual rather than abrupt, so it is often unrecognized.

Kidney essence deficiency — the core of andropause

In Korean medicine, male vitality and stamina originate in kidney essence. As kidney essence is naturally consumed with age, bones weaken (decreased bone density), muscles diminish (sarcopenia), and sexual function declines — all signs of kidney essence deficiency. Overwork, stress, and alcohol accelerate this depletion.

Deer antler velvet and Epimedium (Yinyanghuo)

Below are key herbs frequently used for andropause.

  • Deer antler velvet: The pre-ossified tissue of deer antler tonifies both kidney essence and kidney yang. Studies confirm increases in IGF-1, enhanced hematopoiesis, and musculoskeletal strengthening. It is also a core ingredient in Gongjindan and Gyeongokgo.
  • Epimedium (Yinyanghuo, also called "horny goat weed"): Warms kidney yang and reinforces sexual and physical strength. Pharmacological studies show that its icariin component inhibits PDE5 to improve blood flow.
  • Adjunct herbs: Cuscuta seed (improves semen quality), Eucommia (strengthens lower back and knees), and Lycium berry (tonifies both liver and kidney)

Energy, libido, and muscle — integrated management of co-occurring decline

Andropause is a syndrome rather than a single symptom, so kidney essence must be replenished with herbal medicine while exercise, sleep, and diet are managed together. Resistance training (three sessions per week, focused on large muscle groups) directly stimulates testosterone production, and at least seven hours of sleep is essential for hormonal recovery. Make sure to include zinc-rich foods such as oysters, beef, and pumpkin seeds, and vitamin D-rich oily fish.
