Korean medicine's understanding of infertility
Infertility is diagnosed when natural pregnancy does not occur after one year of attempts. Female factors include ovulation disorders, tubal blockage, and endometrial problems, while male factors account for 30–40%. Korean medicine refers to conception as jongja (種子, "sowing the seed"), and holds that pregnancy occurs when kidney essence (sinjeong, 腎精) is sufficient, the uterus (pogung, 胞宮) is warm, and qi and blood flow smoothly.
Replenishing kidney essence (腎精補充) — Jwagwi-eum and Ugwi-eum
The core of infertility treatment is replenishing kidney essence (sinjeong, 腎精). From a modern perspective, egg quality, endometrial receptivity, and hormonal balance all fall within the scope of kidney essence.
- Jwagwi-eum (左歸飮): Composed of prepared rehmannia (Rehmannia glutinosa, sukjihwang), Chinese yam (sanyak), goji berry (gugija), Cornelian cherry (sansuyu), poria (bokryeong), and licorice (gamcho), this formula replenishes kidney yin (sineum, 腎陰). It is mainly used in the low-temperature (follicular) phase of basal body temperature to support follicle maturation.
- Ugwi-eum (右歸飮): Adds prepared aconite (buja), cinnamon bark (yukgye), eucommia (ducheung), and dong-quai (dang-gwi) to prepared rehmannia, Chinese yam, and Cornelian cherry to reinforce kidney yang (sinyang, 腎陽). Used during the high-temperature (luteal) phase to support corpus luteum function and optimize the implantation environment.
Cycle therapy — phase-specific care for ovulation and implantation
Korean medicine treatment for infertility is most effective when prescriptions are adjusted with the menstrual cycle, an approach known as cycle therapy (jugiyobeop, 週期療法). During menstruation, formulas that activate blood and dispel stasis (hwalhyeolgeoeo, 活血祛瘀) cleanse the uterus. In the follicular phase, formulas that nourish yin and tonify the kidneys (jaeumbosin, 滋陰補腎) build follicles and the endometrium. At ovulation, formulas that activate blood and unblock the channels (hwalhyeoltongnak, 活血通絡) help release the egg. In the luteal phase, formulas that tonify the kidneys and assist yang (bosinjoyang, 補腎助陽) support implantation.
Optimizing the uterine environment — acupuncture, moxibustion, and lifestyle
The better the uterine artery blood flow, the better the endometrial thickness and receptivity. Acupuncture and moxibustion at Guanyuan (CV4), Zigong (Uterus point, EX-CA1), and Sanyinjiao (SP6) significantly increase pelvic blood flow. Limit caffeine to under 200 mg per day, and start folic acid three months before pregnancy. Rather than intense exercise, gentle activities like yoga and walking are more favorable for uterine blood flow.
