Summary

Ninety-six patients with androgenetic alopecia (AGA) were evaluated for the effect of He Shou Wu-based herbal medicine combined with 5% minoxidil compared to minoxidil alone. After 24 weeks, the combination group showed significantly higher hair density increase rate and hair thickness improvement rate, with also decreased scalp sebum secretion.

Combined He Shou Wu-based herbal medicine and minoxidil therapy significantly improves hair growth indicators in male pattern baldness compared to monotherapy.

Seventy-five patients with facial melasma underwent combined therapy with oral Gamisoyo-san and Bombyx mori-containing topical preparation, followed up for 20 weeks. MASI scores significantly decreased in the combination group, with also improved melanin index and skin brightness indicators. Recurrence rate was lower compared to hydroquinone alone group.

Seventy-two patients with dyshidrotic eczema of hands and feet were randomly assigned to compare damp-resolving herbal medicine (modified Pyeongwi-san) versus topical steroids for 8 weeks. The herbal medicine group showed significantly reduced blister occurrence frequency and pruritus, with significantly lower recurrence rates at 8 weeks after treatment completion than the steroid group.

Damp-resolving herbal medicine treatment is effective for both acute symptom relief and recurrence prevention in dyshidrotic eczema.

Summary

Literature on herbal medicine treatment for bullous pemphigoid and other bullous diseases was narratively reviewed. Heat-clearing damp-resolving and blood-nourishing wind-dispelling prescriptions were mainly utilized, with reported steroid-sparing effects and recurrence suppression effects. When combined with herbal medicine, the maintenance dose of steroids could be reduced by an average of 40%.

Key Conclusion

Herbal medicine treatment can play an adjunctive role in steroid reduction and recurrence suppression in bullous diseases.

In the 25 included studies, the combination therapy group showed significant improvement in pruritus VAS scores and itching frequency compared to antihistamine alone groups. Effects were particularly notable in idiopathic pruritus of unknown origin.

Analysis of 14 clinical studies showed that heat-clearing blood-cooling prescription-based herbal medicines had significant effects on improving major rosacea symptoms such as erythema, papules, and pustules. Synergistic effects were observed when combined with topical metronidazole.

Herbal medicine treatment was effective for pain reduction, rash healing promotion, and neuralgia prevention, with no serious adverse reactions reported.

Seventy-four patients with idiopathic chronic pruritus were randomly assigned to blood-nourishing wind-dispelling herbal medicine (Samul-tang combined with Sopung-san) or placebo groups and compared over 8 weeks. The herbal medicine group showed significant decreases in pruritus VAS, pruritus frequency, and sleep disturbance scores compared to placebo. Serum IL-31 levels also significantly decreased in the herbal medicine group.
